Vendor Request Disclosure & Acknowledgment By submitting this vendor request, the vendor acknowledges and agrees that submission of this form does not constitute approval, authorization to begin work, a contract, promise of payment, or commitment by New Zion San Marcos. No goods, materials, services, labor, or other expenses should be provided or incurred on behalf of the church without prior written authorization from an authorized representative of New Zion San Marcos. The church is not responsible for payment for unauthorized work, purchases, additional services, change orders, or expenses that have not been approved in writing. All vendor requests are subject to review and approval. New Zion San Marcos reserves the right to approve or deny any vendor request, obtain additional bids, negotiate pricing or scope, discontinue discussions, or select another vendor at its discretion. Vendors are responsible for maintaining all licenses, permits, insurance, certifications, and other requirements applicable to the services they provide. Vendors may be required to provide documentation, including a completed Form W-9 and proof of insurance, before performing work or receiving payment. Unless otherwise established by a separate written agreement, vendors are independent contractors and are not employees, agents, partners, or representatives of New Zion San Marcos and have no authority to enter into agreements or make commitments on behalf of the church. Any approved work must remain within the authorized scope and amount. Any work or expense exceeding the approved scope or amount must receive additional written approval before it is performed or incurred. By submitting this request, the vendor confirms that the information provided is accurate and acknowledges that they have read, understand, and agree to the terms of this disclosure.
